Illinois Gov. JB Pritzker posted on X that the state has erased $1.4 billion in medical debt for nearly 700,000 residents. The announcement described the relief as helping people avoid years of financial struggle and included a video of the governor speaking about the program. Online users responded by questioning who ultimately covers the cost, with many comments stating that Illinois taxpayers are paying for the debt cancellation.

Medical debt is a leading cause of bankruptcy.

Healthcare should help people get back on their feet, not keep them down with years of financial struggle.

The video shows Pritzker at a podium discussing letters sent to residents under the program. In the clip, he says, “I’ve talked to some of the folks who’ve received a letter like that; people have broken out crying,” and adds that recipients “want to pay their debts” but “with medical debt they often just can’t, it’s too much.”

He describes the automatic relief as meaningful for public service because of its “direct impact on so many people.”

According to the post and related official statements, the Illinois Medical Debt Relief Program has erased more than $1.4 billion in debt for nearly 700,000 residents across all 102 counties, exceeding an initial $1 billion goal while using less than $10 million in state funds.

The state works with the nonprofit Undue Medical Debt to buy unpaid medical bills at a big discount and cancel them for low-income residents. No one has to apply. On average, people got about $1,200 wiped out, and one person had as much as $900,000 canceled.

Responses to the Illinois Medical Debt Relief Announcement

Cook County Board President Toni Preckwinkle backed the program. She said she was glad the state built on what the county had already done and called medical debt relief “one of the most effective investments government can make.”

State senators Lakesia Collins and Mike Simmons also spoke in support. Collins said, “Behind every one of those 700,000 people is a family that can finally breathe again,” and called the program a real difference for communities.

Simmons said the goal was simple, “no one should lose their financial footing because they got sick”, and pointed to the $1.4 billion erased as proof it worked.

On X, many commenters questioned the funding and described the relief as a cost shifted to taxpayers. One wrote, “You can’t just ‘forgive’ debt that isn’t yours to forgive. So in fact, the lawful citizens of Illinois are paying off that debt under your supervision.”

Another said, “Who pays the debt? That’s right. You take money from people who didn’t accrue the debt.”

A third added, “Again, Illinois taxpayers foot the bill for another worthless program.”

Discussion of the medical debt program has continued online and among state officials. Supporters point to the number of Illinois residents who received relief, while critics keep returning to the question of taxpayer funding.
