Lore-wise, Ultra Wormholes are the extra-dimensional passageways that Ultra Beasts use to enter the world of Pokémon GO, according to The Games Cabin. After an Ultra Beast appears, Trainers must gather their most combat-worthy pocket monsters to the Raid and battle the beast in the Gym. Like five-star and Elite Raids, Ultra Beast Raid Bosses typically require three to five Trainers to be defeated.
