How to Complete Malady of the Soul in Diablo 4

Malady of the Soul, Depths of Despair, and Faith in Blood in Diablo 4 are three quests that tie into one. First, you must complete Malady of the Soul, then Depths of Despair, and finally Faith in Blood. Though, you have to do them in order. The first side quest unlocks the next, and the second unlocks the third in the series. To begin Malady of the Soul, you must head to Kyovashad to speak to Sister Octavia of the Cathedral of Light. Keep reading to learn how to complete Malady of the Soul in Diablo 4.

Malady of the Soul in Diablo 4

To begin with, head to Kyovashad, where you can speak with Sister Octavia. You can find Sister Octavia in an alleyway within the city. When you speak to her, she will tell you how she needs some help exorcising a demon that’s taken hold of a boy. She will then lead you into the Frigid Cellar, which can be found around the corner from Sister Octavia.

When you enter the Frigid Cellar, Sister Octavia will instruct you to take her Chalice and place it near the boy while she begins the exorcism. A highlighted area near the boy’s bed will prompt you to place the Chalice when you hover over it with your cursor.

Speak to Sister Octavia, and the exorcism will begin. Take down the various Carvers that the demon spawns and defeat Xul’goth, the demon who once possessed the boy. This will complete the exorcism.

Head back outside to speak to Sister Octavia once more and complete the quest, awarding you 7,680 Experience Points and 1,100 Gold, along with an Herb Stash.

Depths of Despair in Diablo 4

Once you’ve completed Malady of the Soul in Diablo 4, you can begin Depths of Despair by speaking to Sister Octavia once more. This time, she needs help with another possession. Sister Octavia will lead you into the Ominous Cellar nearby. Though, this time around, you will begin fighting immediately. As soon as you arrive inside, there will be Carvers waiting around to take you down. These should be relatively easy to handle and shouldn’t take long.

Now, head back outside to speak to Kvera. Kvera was attacked while you were in the Ominous Cellar. You may also notice that Sister Octavia has started following you as well. After speaking to Kvera, you will be prompted to make your way over to the Kyovashad Sewers and go inside.

Once you’re inside the sewers, your new task is to chase Vasek, the possessed man.  Make your way through the sewers by heading up and to the left, following the main path through the location. Once you get there, he will be on the ground. Rather than talking to Vasek, you need to speak with Sister Octavia to get ready for the exorcism.

Just like you did in Malady of the Soul, you will be expected to place a Chalice down near Vasek in the highlighted area on the ground. Eventually, the demon shows up – an Elite enemy named Demotath. Take him out using shock and flame attacks, then head back outside of the sewers to speak with Sister Octavia again. This will complete Depths of Despair and allow you to begin the next quest, Faith in Blood in Diablo 4.

Rather than taking this quest from Sister Octavia as you did with the two previous quests, you will pick this one up from Petr, who is near the Kyovashad Waypoint. Petr will lead you back to the Ominous Cellar, where you will once again assist with a possible demonic possession. Head inside and speak to Sister Octavia, who is injured and lying on the bed that you found the boy on in Malady of the Soul.

Next, you’ll have to head over to Margrave, where you will speak to Priest Matvey. You can find him near the pyre. You will speak to him about getting a Holy Chalice so you can save Sister Octavia. He’ll give you one, but you’ll have to consecrate it first by filling it with bandit blood.

Looking at the map, you should see a large highlighted area near Margrave. Head to this location and take out as many bandits as you can. Every time you defeat one, the chalice will fill a little bit until it is completely full.

Once the Holy Chalice is full of bandits’ blood, head back into Margrave and interact with the pyre. This will add a piece of pyre wood to the Holy Chalice, and it will be ready to use. Now, head back into the Ominous Cellar to begin the exorcism and save her from the demon.

This time, you will fight Demotath again, the demon who was possessing Vasek in Depths of Despair, and complete the Faith in Blood in Diablo 4.
