Now that the Echo of Varshan and the Malignant Burrow has been integrated into the main game from its Seasonal roots, you’re going to need luck not just in looting items but also in looting body parts. The Gurgling Head in Diablo 4 is one such part. You need it to gain access to the Malignant Burrow right next to the Tree of Whispers. It just so happens that the Gurgling Head in Diablo 4 is more elusive than other body parts. Here’s how to obtain it.

Where to Get the Gurgling Head in D4

Hopefully, you’ve started the Season with a character that skipped the campaign because unlocking the Tree of Whispers can be a pain otherwise.

You’ll want access to the Tree of Whispers for this endeavor if you want to make it easy. The Tree of Whispers has a chance to reward you with the Gurgling Head if you complete its bounties. This is the most consistent way to get the Gurgling Head. There are also other requirements, so here’s a step-by-step process for you.

  1. Ensure that you have access to the Tree of Whispers.
  2. Increase your game’s difficulty to World Tier 3 or World Tier 4.
  3. Finish Whispers of the Dead bounty.
  4. Head back to the Tree of Whispers for the reward cache.
  5. Open the reward cache and you might get rewarded with a Gurgling Head.
  6. If not, repeat steps 3, 4, and 5 again.

However, you can also try out your luck outside of finishing bounties for the Tree of Whispers.

Enemies like the Grotesque Debtor or other Elites have a chance of dropping a random Malignant Body Part. You might find these enemies in Legion events, after World Bosses, or during Helltides.

But given that there are three Malignant Body Parts and certain enemy spawn rates can be uncommon, you’re be playing a lot with your luck. The Tree of Whispers simply has better rates. Still, it wouldn’t hurt to keep an eye out for Grotesque Debtors or Elites in the open world.

Once you have the Gurgling Head along with the Trembling Hand and Blackened Femur, you can head over to the Malignant Burrow right beside the Tree of Spirit.

This is a tunnel dungeon with a pedestal at the end where you can deposit the Malignant Body Parts to summon the Echo of Varshan. Note that simply having the Gurgling Head in Diablo 4 alone isn’t enough as you need other body parts as well.

Diablo 4 is available on PC, PlayStation, and Xbox.
