This is because the best way to play him is to balance his use of perks with his abilities. Our guide highlights some of the best builds for The Doctor so you can decimate the Trial in Dead by Daylight.
Best Builds for The Doctor in Dead by Daylight
Herman Carter, AKA The Doctor, comes courtesy of CHAPTER 4: Spark of Madness. Even with all the updates and tweaking, the Killer remains one of the more frustrating ones to play against. Of course, he has to be in the right hands. This comes with using the right perks that complement his powers.
So, before we get to the best Dead by Daylight builds for The Doctor, let’s take a look at his unique perks:
- Overwhelming Presence: Survivors in your Terror Radius suffer from inefficiency. When they use an Item, its depletion rate increases by 80/90/100%.
- Monitor & Abuse: Base Terror Radius increases by 8 meters. If you aren’t in a chase, your Terror Radius reduces by 16 meters. Meanwhile, your Field of View increases by 3/5/10 degrees.
- Overcharge: Activates once you damage a Generator, and a Survivor interacting with it has a difficult skill check. If failed, there’s a progression loss of 3/4/5%. Additionally, the Generator sees an increase in regression speed from 75% to 200% for 30 seconds.
Even when you have the best builds for The Doctor in Dead by Daylight, his power can make or break a game.
He has Carter’s Spark. This uses Shock Therapy and Static Blast to drive Survivors into Madness. Once the effect is applied, Survivors cannot lose it. They can only regress back to Madness I if they snap out of it. There are three total Madness levels that can cause issues like Madness Skill Checks, Hallucinations, and reveal their position.

Best Beginner Build for The Doctor
You can use the general Killer perks as well as perks from the base Killers to create a strong build for your Doctor. For this build, bring Overcharge and Overwhelming Presence from The Doctor to mess with Survivors and Generators. To continue your assault against them, add these perks:
- Jolt (General): When you down a Survivor with a Basic Attack, Generators within 32 meters explode and regress. They have a 6/7/8% progression penalty.
- Whispers (General): If at least one Survivor is within 48/40/32 meters, you hear whispers from The Entity.
Out of the best Dead by Daylight builds for The Doctor, this is a solid start. It can help you learn how to use The Doctor, keep Survivors on the move, and slow down Generators.
An add-on like “Order” – Class I is useful. It not only lowers the cooldown of your Static Blast, but it further messes with Survivors afflicted with Madness.
Best All-Perks Builds for The Doctor in Dead by Daylight
Although you can have a reliable build with basic perks, The Doctor excels when you have access to all the Killer perks. In fact, the right builds can lead to some funny ragequitting.
Let’s start with a well-known Doctor build: the skill check build. This is notorious for aggravating Survivors, especially Solo players. For you, it’s sheer joy. Bring Overcharge from The Doctor and use these perks with it:
- Unnerving Presence (The Trapper): Impacts Survivors healing or repairing within your Terror Radius. There’s a 10% increase in trigger odds of skill checks and a 40/50/60% decrease in the success zone of skill checks.
- Merciless Storm (The Onryō): When a Generator reaches 90%, Survivors fixing it have constant skill checks. If a check is missed or they stop, the Generator is blocked for 16/18/20 seconds.
- Hex: Huntress Lullaby (The Huntress): Survivors get a 2/4/6% regression penalty on failed skill checks. The hex gains a Token each time you hook a Survivor.
- 1 Token: -14% time between warning sound and skill checks
- 2 Tokens: -28% time between warning sound and skill checks
- 3 Tokens: -42% time between warning sound and skill checks
- 4 Tokens: -56% time between warning sound and skill checks
- 5 Tokens: removes the warning sound

This next build encourages you to be an aggressive Doctor. This also expects your Totem to remain active, but if you properly use your power, you can build up Madness and keep Survivors off your Totem. You also want to slow Survivors down, so we have perks for that too:
- Hex: Devour Hope (The Hag): If you’re 24 meters away from a rescued Survivor, the hex receives 1 Token.
- 2 Tokens: 10 seconds after you hook a Survivor, you will gain 3/4/5% Haste for 10 seconds
- 3 Tokens: Survivors are permanently Exposed
- 5 Tokens: You can kill all Survivors with your hand
- Hex: Undying (The Blight): Reveals the auras of Survivors within 2/3/4 meters of any Dull Totem. If another hex Totem is blessed, said hex transfers to the Undying Totem.
- Thrilling Tremors (The Ghost Face): When you pick up a Survivor, The Entity blocks any Generator not being repaired for 16 seconds. Their auras are highlighted in white.
- Pop Goes the Weasel (The Clown): Activates for 35/40/45 seconds after you hook a Survivor. When you damage a Generator, it regresses -20% of its current progression.
