Near the beginning of Baldur’s Gate 3, you meet Mayrina’s brothers and Auntie Ethel. If you play your cards correctly, you will receive a quest called “Save Mayrina.” To complete this quest, you must face Auntie Ethel, who is a Hag, and defeat her boss form. Then you will need to know how to get Mayrina out of the cage before the cage burns. And after saving Mayrina, you can get an additional side quest with an excellent reward in BG3.

After you’re done with Mayrina’s brothers, head to the Riverside Teahouse below the Blighted Village. Depending on the dialog options you choose, different things will happen. For example, if you go through the choices about the parasite, you’ll be fine, but if you take an offensive approach to Mayrina, things will escalate. But it doesn’t matter too much which path you take, either way, you can fight Auntie Ethel’s boss form in BG3 and get Mayrina out of the cage.

Auntie Ethel teleports away at the end of your conversation and puts Mayrina in her cage. To get Mayrina out of the Cage in Baldurs Gate 3, go to the Fireplace at the back of the room, and put out the fire. You can move from the Fireplace to the hidden area behind it. Go down, and you will see the Gnarled Door, but you will not be allowed to pass. To get through this door, you must wear one of the Whispering Masks on the table. However, it is enough if one person in the party wears it. You can take it off after you pass through the door.

After killing the enemies inside, take the secret path behind the waterfall, and find Mayrina and Auntie Ethel in BG3. After a dialog with Auntie Ethel, the fight will start, and Auntie Ethel will start burning Mayrina’s cage. You must kill Auntie Ethel before the cage burns down and use the orange Control Orb in the center of the room to bring the cage down.

After saving Mayrina in BG3, you can pick up the Bitter Divorce wand in the Acrid Workshop at the back side and choose whether or not to resurrect Mayrina’s husband. You can find Mayrina again by passing through Mushroom Circle at the back of the Acrid Workshop.
