Yes, Diablo 4 has a couch co-op mechanic. Before you get into any online aspects of the game, you will need to play through a few early key moments. The game opens with a cinematic that sets its dark tone before letting you customize your character. After selecting your class and gender, and curating this to your taste, you will play the Prologue. After killing some creatures and completing a few minor quests, you will eventually reach the city of Kyovashad in the Fractured Peaks. At this point, you can have someone sit next to you and jump right into the action.
Through the game menu, you can invite people into your server. You can also use the Battle.net launcher for PC players. On consoles, you have the Xbox and PlayStation menus to get someone into your party. Even if couch co-op in Diablo 4 is only for PlayStation and Xbox players, all other multiplayer functionalities are available for PC players. Party sizes are capped at four.
By having other players in your game, the difficulty scales to your party size. Enemies deal more damage and have more health. They are scaled to each individual player, so it does not matter too much if there is a level gap between you and other party members. If you work together and have a good combination of classes, then you will make it out alive. When you kill monsters or complete quests, rewards will be individualized, so nobody will steal your stuff.
